HOW DO I ORDER?
Email your art or logo to art@plastilam.com and fill out the order sheet below. (we will send you a PDF proof of your card for your approval before the production of your card order). Note: You will not be billed until you have approval the card design

1) Select your gift card order quantity.
Standard card pricing. Size is 2 1/8 X 3 3/8 X .030 thickness (prints up to 4p/1) includes mag stripe
if required. Add $45.00 set-up plus:

	250 Gift Cards $275.00			500 Gift Cards $345.00
	1,000 Gift Cards $590.00			2,500 Gift Cards $1,225.00
	5,000 Gift Cards $1.950.00			10,000 Gift Cards $2,450.00

2) Add to have cards numbered or magnetic stripe encoded.
Add set-up of $45.00 plus:

250 Cards: add $72.50	500 Cards: add $95
1000 Cards: add $120	2500 Cards: add $250
5000 Cards: add $400	10,000 Cards: add $700

3) Shipping: UPS ground Shipping is $29.00 to any address in continental USA.
